How do I download and fill out a PDF form

To download and open a PDF form, you need to have Adobe reader 10 or higher installed. Mobile devices such as iPads, tablets and mobile phones can’t be used for our forms.

Downloading and opening PDF file

  • Find the link of the file you want to save (you may need to go back to the previous page to find it)

  • Click the link

  • Go to upper right corner, click the download icon

  • Choose the location on your computer where you would like to save the file

  • Select “Save”

  • Use Windows Explorer to get to the location where you saved the file

  • Right click on the PDF file

  • Select “Open with”

  • Select “Choose program”

  • Select “Adobe Acrobat Reader”

  • Make sure to check the box that says “Always use this program to open these files”

  • Select “OK” 

How to fill out a PDF form

  • Open the saved PDF file

  • Use the mouse to select (click on) a specific field.

  • Use the Tab key to move through the fields. Shift + Tab will go to the previous field.

  • Use the space bar or click the mouse button to mark or unmark a box.

  • After filling out the form and reviewing it, select the Print button on the form, or press Ctrl-P on a PC, or Command-P on a Mac.

  • You can save the form as an electronic copy for your records or you can click on the Reset button to clear all the entered data to protect your information from any other users of this computer.

  • Sign and email your completed form to

  • Opening the PDF file may take a little bit longer to allow the enhanced features to load.
